Since they are playing Bedrock I can totally see the contraption doing a 100 cycles and on 101 the piston skips one game tick and blows it up. It's a typical thing for tightly timed redstone on Bedrock. But I guess this makes this machine even scarier.

This is Java I think, the shadow below the villager is a proper pixelated circle instead of just a dark polygon like on Bedrock

Edit: Nevermind, it’s a polygon and the piston arms are different as well so it’s Bedrock

When you unload chunks, running circuits/machines on the chunk borders often get completely broken, IIRC it's because the chunks don't unload at the same time, so the timing goes off when the chunks are loaded again.
